Candidate Application Must have a sponsor form and $60 deposit included before mailing.
Sponsor Form Must accompany the Candidate Application with a $60.00 deposit, If you are requesting the Dibert Fund the application must include a $75 deposit. Note: It seems as though some computers have printer settings that are printing out this form very small.
